All incisions start by going around the areola; then, depending on the task at hand, there may be an additional incision along the breast crease, or vertically down from the areola, or horizontally along the breast crease.
The process may include removing excess breast skin in order to counter the loss of elasticity. The nipple and areola may be moved up to somewhere near their former height; and the underlying breast tissue is lifted and contoured to improve both appearance and firmness. Any sutures along the breast crease will be concealed. Sutures along the outer surface of the breast will be visible, but will be unobtrusive and will fade over time.
Once the swelling fades, you’ll have a new figure to appreciate. You should know that repositioning the nipple may very likely reduce sensation in that area of your breast. It is also probably a procedure that you should postpone until you are done with childbearing, since it can impact the ability to breastfeed.
